IT Band Symptoms: How to Tell If You Have It

Every symptom of it band runners should recognize — from the earliest warning signs to when to stop and see a clinician.

What IT Band Feels Like

IT band syndrome (ITBS) is irritation of the tissue at the outside of the knee where the iliotibial band crosses the femur. The band itself doesn't 'tighten' — the underlying tissue gets compressed and inflamed from repeated knee flexion under poor hip control.

  • Sharp pain on the outside of the knee, usually starting at a predictable distance
  • Pain worse on downhills
  • Tightness in the outer thigh after running
  • Pain that resolves quickly with rest, returns with running
  • Sometimes a snapping or rubbing feel at the knee

Early Warning Signs

Catching it band at this stage is the difference between a week of reduced training and a multi-week shutdown.

  • Outer-knee tightness after long runs
  • Discomfort on downhill running
  • Tightness that goes away walking

When to Stop and See a Clinician

  • Sharp outer-knee pain that stops a run
  • Pain on every step
  • Pain when descending stairs
