Hip Pain On Impact: Why It Hurts to Land

What this pattern usually means
Pain that spikes specifically on impact is a yellow-to-red flag. Bone, joint surface, or stress reactions hurt with vertical load. This is the pattern that most often precedes stress fractures.
What to do today
- Stop running today — walk back if needed
- Use the single-leg hop test (gently): pain = stop
- Reduce weight-bearing activity for 5-7 days
- If symptoms persist 7+ days, get imaging

Common causes of hip pain

Could it be one of these injuries?
Piriformis syndrome is irritation or compression of the sciatic nerve as it passes near the piriformis muscle deep in the glute. Symptoms can mimic sciatica from a lumbar disc, which makes diagnosis tricky. True piriformis syndrome is overdiagnosed; deep gluteal pain is often actually proximal hamstring tendinopathy or low-back referral.
A hip flexor strain involves the iliopsoas or rectus femoris — the muscles that lift the leg forward. Most common in sprinters, hurdlers, and runners who spike speed work, but also seen in distance runners with high mileage and lots of sitting outside of training.
