Piriformis Symptoms: How to Tell If You Have It

What Piriformis Feels Like

Piriformis syndrome is irritation or compression of the sciatic nerve as it passes near the piriformis muscle deep in the glute. Symptoms can mimic sciatica from a lumbar disc, which makes diagnosis tricky. True piriformis syndrome is overdiagnosed; deep gluteal pain is often actually proximal hamstring tendinopathy or low-back referral.

  • Deep ache in the buttock that worsens with sitting
  • Discomfort that may radiate down the back of the thigh
  • Pain climbing stairs or hills
  • Tightness through the deep hip rotators

Early Warning Signs

Catching piriformis at this stage is the difference between a week of reduced training and a multi-week shutdown.

  • Tightness deep in the glute after long runs
  • Discomfort with prolonged sitting
  • Hip stiffness after speed work

When to Stop and See a Clinician

  • Numbness or tingling down the leg
  • Pain that stops you running
  • Weakness in the foot or leg
